Credit Union Forms

Below are various forms you may download and print. Note: For additional information on completing the forms below, please contact the credit union.

• Address Change Form
  Upon completion, please mail the form in to the Credit Union at P.O. Box 70009, Houston, TX 77270-0009 and drop it off at any branch location. Note: You must be a primary or joint owner on the membership’s primary savings account to request a change of address.

• Cardholder Dispute Form (HTFFFCU)
  To dispute a transaction made on your HTFFFCU Debit MasterCard, VISA or MasterCard credit cards.

• Cardholder Affidavit Form (CUNA Mutual)
  Complete this Cardholder Affidavit Form for the purpose of establishing fraudulent use of your Credit/Debit/ATM card(s).

• Credit Card Authorized Signers Form
  Complete this form to add authorized signers to your VISA or MasterCard credit card. Upon completion, please fax this form to Card Services at (713) 715-6192 or mail it to the Credit Union at the PO Box address.

• Credit Card Authorization Agreement for Pre-Authorized Payments
  Complete this form to authorize the Credit Union to initiate withdrawals from your VISA or MasterCard credit card. Upon completion, please fax this form to Card Services at (713) 715-6192 or mail it to the Credit Union at the PO Box address.

• Credit Card Balance Transfer Form
  Transfer high balanced credit cards to your VISA or MasterCard. No Balance Transfer fee incurred!

• Credit Card Closure Form
  To close your VISA or MasterCard credit card, complete this form. Upon completion, please fax this form to Card Services at (713) 715-6192 or mail it to the Credit Union at the PO Box address.

• Cross-Member Transfer Form
  Conduct cross-member deposits between accounts using the CU Online and PATT automated systems.

• CU Online Enrollment Form
  Complete and return this form to sign up for CU Online.

• Daily Limit Increase Form
  Complete this form to apply for a daily limit increase on your ATM or Debit MasterCard. Upon completion, please fax this form to Card Services at (713) 715-6192 or mail it to the Credit Union at the PO Box address.

• Member Grievance Form
  To file a grievance with the Credit Union.

• PATT Enrollment Form
  Complete and return this form to sign up for Personal Automated Telephone Teller (PATT) service.

• Reopen Card Request
  Request to reopen your Debit MasterCard, ATM or VISA or MasterCard credit cards. Upon completion, please fax this form to Card Services at (713) 715-6192 or mail it to the Credit Union at the PO Box address.

• Request to Reset CU Online PIN
  Forgot your CU Online PIN? Complete this form and fax back to the Credit Union at (713) 864-4262

• Wire Transfers
  Wire transfers allow you to send and receive guaranteed funds to and from any one around the country. A fee of $10.00 is charged for all outgoing wire transfers. For further information, limitations and details, please click here.
